NREMT patient assessment skill sheets

The National Registry has four patient assessment sheets: a medical and a trauma sheet for EMTs, and an advanced-level pair for AEMTs and paramedics. Here is how they compare, and what they have in common.

Transport is the time limit for initiating or calling for transport, a critical criterion on every sheet. The National Registry's AEMT and paramedic psychomotor exam ended on June 30, 2024, but the advanced-level sheets still define a complete assessment at those levels; the AEMT and paramedic pages cover them.

Common ground

How every sheet is scored

  • Points come one at a time. Each step marked (1 point) is scored on its own, so a 3-point line can earn 0, 1, 2 or 3.
  • Critical criteria override points. Any one of them fails the station, however many points you earned.
  • There is a clock. Calling for transport too late is a critical criterion: 15 minutes on the medical sheets, 10 on trauma.
  • Say what you are doing. An examiner can only score what they see or hear, so state each assessment as you do it.

The National Registry publishes the official sheets at nremt.org. Medceptor is independent and isn't affiliated with or endorsed by the NREMT.

Questions

Asked & answered.

Do the skill sheets list a passing score?

No. Each sheet lists the points for every step and a set of critical criteria, but no minimum score. Any one critical criterion fails the station regardless of points.

Are the AEMT and paramedic sheets different from the EMT sheets?

The trauma sheets share the same structure and the same 42 points. The advanced-level medical sheet is worth 48: it adds a Diagnostics line for the ECG monitor, AVPU as a vital sign and a transport re-evaluation, splits reassessment into four lines, and drops the verbal report line.

Where can I get the official sheets?

The National Registry publishes them at nremt.org. Medceptor's pages walk through the EMT sheets' wording, but before an exam always check the current official version.
