Critical criteria
What fails the station outright
Any one of these fails the station, however many points you earned.
- Failure to initiate or call for transport of the patient within 15 minute time limit
- Failure to take or verbalize appropriate PPE precautions
- Failure to determine scene safety before approaching patient
- Failure to voice and ultimately provide appropriate oxygen therapy
- Failure to assess/provide adequate ventilation
- Failure to find or appropriately manage problems associated with airway, breathing, hemorrhage or shock
- Failure to differentiate patient's need for immediate transportation versus continued assessment or treatment at the scene
- Performs secondary examination before assessing and treating threats to airway, breathing and circulation
- Orders a dangerous or inappropriate intervention
- Failure to provide accurate report to arriving EMS unit
- Failure to manage the patient as a competent EMT
- Exhibits unacceptable affect with patient or other personnel
- Uses or orders a dangerous or inappropriate intervention
The printed sheet lists the dangerous intervention criterion twice, in two wordings. Either one fails the station.
Scoring
How points are won and lost
- Each point stands alone
- History of the present illness is eight separate points: one each for onset, provocation, quality, radiation, severity and time, and two for clarifying questions about associated symptoms. Saying "OPQRST" earns none of them; asking each one does.
- Scene safety comes first
- The critical criterion reads "before approaching patient", so checking scene safety later still fails the station.
- Oxygen is said and done
- The critical criterion is to voice and ultimately provide appropriate oxygen therapy. Naming it and never applying it can still fail you.
- Make the call yourself
- Priority, transport and the field impression score when you state your own decision. Asking the examiner whether the patient is high priority isn't a decision.
- Finish with the report
- The last line is an accurate verbal report to the arriving EMS unit, and failing to give one is also a critical criterion.
Practice
Practice E202 on Medceptor
EMT medical scenarios on Medceptor are scored on these 42 points. An AI patient answers your questions, a preceptor gives you findings and vitals when you check for them, and your debrief marks every line earned or missed and names any critical criterion that was marked.
Your performance is evaluated by an autonomous AI grading system. Scores are for instructional self-assessment only and are not predictive of the NREMT, the NCLEX or any other certification exam.
Questions
Asked & answered.
How many points is the NREMT medical patient assessment?
42. The biggest blocks are history taking (13 points: 8 for the present illness and 5 for past medical history) and the primary survey (10 points).
What is the time limit?
You have to initiate or call for transport within 15 minutes. Missing it is a critical criterion, so it fails the station on its own.
Is E202 used for AEMT and paramedic students?
No. AEMT and paramedic students use the 48-point advanced-level medical sheet (P302), which adds a Diagnostics line for the ECG monitor and a longer reassessment.
How can I practice E202 without a partner?
Run EMT medical scenarios on Medceptor. An AI patient and preceptor play the call, and you're graded on the sheet's 42 points with every line marked earned or missed. Three scenarios are free.
